What you'll do

• Conduct nutrition assessments, reassessments, and follow-up evaluations using the Nutrition Care Process
• develop, implement, monitor, and modify individualized nutrition care plans
• provide medical nutrition therapy to diverse inpatient populations
• assess enteral and parenteral nutrition support
• identify malnutrition risk
• educate and counsel patients and families
• interpret clinical information
• collaborate with healthcare professionals
• document care in the electronic medical record
• participate in rounds, conferences, discharge planning, and quality initiatives as appropriate.
