The role provides routine direct care according to each tenant’s service plan, supporting daily living, independence, decision-making, and physical, mental, and psychosocial well-being as part of an interdisciplinary care team.

What you'll do

• Provide routine direct care and assistance with activities of daily living under licensed nurse or clinical manager supervision
• follow tenant service plans and applicable nursing assistant standards
• apply basic nursing, personal care, and restorative skills while attending to residents’ mental health, social service, and cognitive needs.
