What you'll do

• Serve on the interdisciplinary team to shape care plans
• collaborate with participants, families, and staff
• conduct initial and periodic health assessments
• coordinate supplies, equipment, and services
• visit participant homes
• participate in the on-call rotation
• and lead hospital-to-home care transitions and discharge planning.
