What you'll do

• Perform initial and follow-up evaluations and assessments
• engage veterans and PACT team members in treatment planning
• provide individual therapy and evidence-based psychotherapy as appropriate
• deliver brief interventions focused on behavior activation and short-term needs
• provide emergency and crisis intervention services
• coordinate referrals, family conferences, and case management
• enter patient information and complete clinical reminders
• participate in program planning, performance improvement, and veteran feedback activities.
