What you'll do

• Conduct initial and ongoing assessments
• determine eligibility and appropriate levels of home health care
• develop, implement, and document plans of care
• educate patients and families
• initiate preventative and rehabilitative nursing procedures
• apply infection-control precautions
• provide clinical direction to home health aides and LPN/LVNs
• document progress
• and communicate changes in patient needs to physicians and care team members. Requirements include a current RN license, driver’s license, CPR certification, and valid auto insurance
• two years of home health RN experience and HomeCare HomeBase experience are preferred.
