The Home Health Field Registered Nurse (RN) role is an onsite, full-time position in Gilmer, TX. Compensation is not specified. The nurse supports Choice Health At Home’s delivery of patient-centered care in patients’ homes. The role exists to provide continuity of care outside traditional clinical settings. The RN assesses patients, develops and documents care plans, coordinates with patients, families, physicians, and interdisciplinary staff, and supports recovery, chronic disease management, and patient independence.
What you'll do
- Assess patients’ physical, functional, psychosocial, and environmental needs
- determine eligibility and appropriate service levels
- develop, implement, document, and revise plans of care
- educate patients and families on self-care, nutrition, and dietary needs
- provide preventive and rehabilitative nursing procedures
- apply infection-control precautions
- direct Home Health Aides and LPNs/LVNs
- document progress
- communicate changes and outcomes to physicians and care team
- monitor cases for third-party payer compliance.

Why this role
