Housing Case Manager

What you'll do

• Help clients obtain permanent housing and address barriers
• provide advocacy, crisis intervention, and counseling
• develop and monitor service plans
• maintain reports and case records
• facilitate groups and workshops as needed
• build service-provider linkages
• transport clients or arrange transportation to appointments as needed
• coordinate follow-up and outcome measurement
• provide life-skills training
• conduct home visits
• support access to entitlements.
