Remote MDS Coordinator, Minnesota (PRN)

The Remote MDS Coordinator, Minnesota (PRN) role is remote and supports six Eden Senior Care facilities in Minnesota. Compensation is $36–$43 per hour. Eden Senior Care manages skilled nursing, rehabilitation, and assisted living communities; occasional onsite availability is valued but not required. The coordinator manages MDS and reimbursement-related work, supporting accurate assessments, care planning, documentation, and quality processes across the facilities. The role helps interdisciplinary teams meet clinical, regulatory, and reimbursement requirements.

What you'll do

• Schedule, set up, and complete MDS assessments
• manage admission authorizations and updates and communicate with families, insurers, and case managers
• perform ICD-10 coding
• participate in care plan development and review
• discuss care plans, insurance, and discharge needs with residents and families
• educate the interdisciplinary team on MDS requirements and skilled care services
• support documentation through education and audits
• support quality measures and QAPI
• conduct chart reviews and assist during surveys.

Why this role

As-needed remote position. Requires an active Minnesota RN license; Illinois and Wisconsin RN licenses are a plus. Three or more years of MDS experience is preferred, along with knowledge of ICD-10, the RAI Manual, Medicare Chapter 8 guidelines, and PointClickCare.
