WelbeHealth’s interdisciplinary team provides comprehensive primary care and social services to help frail seniors live safely in the community. The Social Worker maintains a participant caseload and coordinates psychosocial services with the interdisciplinary team. Through assessments, care planning, counseling, case management, and care transitions, the role helps participants access support and remain safely and independently in their homes.

What you'll do

• Conduct psychosocial assessments and participate in interdisciplinary care planning
• deliver and document social work interventions
• arrange resources and services
• assist with care transitions
• provide individual and group counseling and case management
• coordinate care conferences
• support discharge planning when participants leave the program.
