What you'll do

• support continuity of care across nursing units
• identify and resolve unit-level problems
• communicate resident status to nursing leadership
• contribute to discharge planning
• coordinate charge, treatment, and CNA assignments
• monitor care and treatment delivery
• support admissions during off hours
• document resident progress.
