What you'll do

• Provide nursing care under the client’s plan of care, including health and psychosocial evaluation, monitoring, health promotion, service coordination, teaching, and direct care. Monitor and report changes in symptoms or behavior
• communicate with physicians and specialists
• educate patients and families
• facilitate referrals
• maintain timely clinical records and notes
• provide advice and emotional support
• participate in case conferences and discharge planning.
