What you'll do

• Provide personal care, habilitative, and supportive services under individual plans
• assist with activities of daily living, hygiene, meals, dressing, toileting, exercise, and ambulation
• support medication assistance as applicable
• maintain clinical records
• communicate changes in participant status
• assist with household tasks, community activities, socialization, communication, safety, and habilitation skills
• maintain CPR and First Aid training.
