What you'll do

• Meet referred patients and families in person or by phone
• screen for unmet needs
• complete and track applications for Medicaid, CHIP, Marketplace, WIC, SNAP, and clinic assistance
• refer to community agencies and behavioral health providers and confirm contact
• follow up on cases
• provide services in Spanish and interpret during case-management conversations
• coordinate with clinic teams
• document contacts and referrals
• follow HIPAA, safeguarding, and mandatory reporting requirements
• travel between southern clinics.
