What you'll do

• Coordinate with colleagues to assess, plan, implement, and evaluate patient care plans
• administer wound care and monitor patients for improvement or decline
• document and report changes in patient condition
• discuss observations with providers
• use electronic health record systems such as WoundAlly
• visit patients at homes and facilities in the listed service areas.
