Physical Therapist (Home Care, PRN)

Provides evaluation and physical therapy treatment in accordance with agency standards, laws, and regulations. Analyzes and documents patients and family's response to interventions. Evaluates measurable progress toward goals and revises the plan of care. Establishes and documents teaching plan for patients/families.
