What you'll do

• Manage a caseload of around 45 individuals across Luzerne, Wyoming, and Lackawanna Counties
• match clients with direct care staff based on needs, skills, compatibility, and availability
• review, implement, and monitor existing Individualized Support Plans under Pennsylvania ODP waiver guidelines
• submit case evaluations, administrative updates, and compliance reports
• coordinate with county entities, medical professionals, and families
• conduct regular field visits
• maintain electronic case records, progress notes, and service authorizations.
