Its work supports continuity of care and compliance with program benchmarks.

What you'll do

• Track required medical case management activities and deadlines
• review completed case activities
• monitor examinations, Annual Case Reviews, and other required services
• complete individualized treatment plans
• follow up on pending assignments and barriers
• request and review medical records
• prepare and distribute case management packets
• coordinate with providers, caregivers, and caseworkers
• maintain case notes and HealthWorks Evolve Portal data
• complete SACWIS and DCFS reporting tasks
• monitor benchmarks and prepare production reports.
