What you'll do
- Conduct consultation and admission visits
- assess hospice appropriateness and review medical records as requested
- document assessments and medications and initiate care plans
- plan and implement immediate interventions on admission day
- coordinate medications and durable medical equipment for same-day delivery
- coordinate with attending physicians and the Hospice Medical Director
- communicate admission information to assigned staff, including case managers and social workers
- perform routine or unscheduled nursing visits when required and provide backup to field staff when there are no admissions
- educate patients and families. Participate in weekend on-call rotation, holidays, and after-hours coverage as needed.
