What you'll do

• Provide direct patient care and assessment
• oversee care plan development and implementation
• identify physical and psychosocial signs and symptoms
• administer medications and treatments as needed under authorized orders
• evaluate patient responses and outcomes
• educate patients and families about palliative and end-of-life care
• advocate for patient and family choices
• document patient condition, treatment, and response
• place referrals and liaise with hospitals and nursing facilities
• work a weekend every eight weeks and participate in holiday rotation.
