What you'll do

• Meet with patients and families to explain hospice services
• coordinate patient admissions and ongoing plans of care
• assess physical condition according to the plan of care and determine or revise medication and durable medical equipment plans
• provide emotional support and instruction in activities of daily living
• complete daily paperwork and charting
• evaluate hospice appropriateness and complete admission visits and documentation
• communicate significant findings to the case manager.
