What you'll do

• Provide remote patient coaching and care
• develop care plans with patients, caregivers, and providers
• monitor outcomes, care-plan adherence, and progress and facilitate changes as needed
• educate patients about chronic conditions, preventive care, and physician directives
• conduct telephone outreach in English and Spanish
• facilitate enrollment in CCM, RPM, and BHI programs
• document accurately
• communicate with primary care providers and follow up on inquiries, care transitions, and referrals.
