The coordinator supports the Program Director with daily program operations and service coordination, ensuring services follow each person’s Individual Plan. The role helps maintain compliant, well-coordinated supports through staff training, plan follow-up, accurate records, and communication with individuals, families, agencies, and internal teams.

What you'll do

• Ensure Individual Plans are implemented
• report changes in individual needs and recommend plan revisions
• train support service workers on Individual Plans
• attend team meetings as needed
• coordinate with departments
• maintain consumer management information and payroll inputs
• ensure regulatory and policy compliance
• support worker engagement and onboarding
• provide emergency coverage and participate in the on-call rotation.
