What you'll do

• Manage a caseload of youth in grades 9–12
• provide up to one year of post-high-school support with education, career, and basic living plans
• build relationships with school personnel, caregivers, social workers, and partners
• analyze youth data to inform educational plans
• coach youth in problem-solving, self-advocacy, and self-determination
• refer youth to resources
• facilitate inter-agency meetings
• maintain accurate case records.
