What you'll do

• Conduct initial face-to-face in-home assessments
• develop written Plans of Care
• reevaluate and update plans when client conditions change or at required intervals
• perform and document supervisory visits at least once every 90 days
• report changes in condition, safety hazards, or suspected abuse or neglect
• participate in Virginia Department of Health inspections and clinical audit readiness.
