What you'll do

• Provide personal care, meal preparation, and daily routine support
• assist individuals with independence, socialization, and daily living goals
• support community outings and activities
• administer medications according to care plans and company policies
• monitor health, safety, and well-being
• respond appropriately in crisis situations
• complete documentation accurately and on time
• follow individualized care plans
• communicate with team members and participate in trainings and meetings.
