What you'll do

• Conduct nutrition assessments for new admissions and quarterly, with more frequent reviews for high-risk residents
• develop, implement, and monitor individualized care plans
• provide nutrition education and counseling
• collaborate with physicians, nurses, therapists, and other care-team members
• maintain documentation and regulatory compliance
• monitor resident weights
• participate in quality improvement
• conduct monthly kitchen sanitation audits and recommend corrective actions
• review and approve menus semi-annually for USDA guideline compliance
• perform other tasks within scope of practice as needed.
