Clinical Care Manager

Why this role

Requires a bachelor’s degree in a related field, at least five years of care coordination or case management experience, California residency, a valid California professional BSW, MSW, LVN, or RN license, Medi-Cal eligibility experience, CPR certification, and a California driver’s license with personal vehicle and insurance.
