The Outpatient Mental Health Therapist role is onsite in Wexford, PA. Compensation is $65,000–$85,000 per year, with an opportunity to earn an additional $7,000 or more in incentives. Revelations Counseling & Wellness describes a collaborative, integrated care model combining counseling, nutrition, and wellness services. The therapist provides individualized outpatient therapy and develops plans that support client stability, healing, and wellness. The role contributes to coordinated care through case consultation, discharge planning, timely documentation, and collaboration with clinical and wellness colleagues.

What you'll do

• Provide customized individual and group outpatient therapy
• develop and implement crisis, safety, relapse-prevention, and wellness plans
• participate in case consultations and discharge planning
• complete session notes, documentation, and treatment plans
• and collaborate with physicians, nurse practitioners, dietitians, administrative staff, personal trainers, and fellow counselors.
