Social Worker, Lakeside

What you'll do

• Complete and document social work and psychosocial assessments within three days of admission or readmission
• complete required forms and care conference coordination
• complete MDS sections and appropriate CAAs
• review and update care plans
• monitor behavior logs and high-risk residents
• coordinate with psychiatric and ancillary providers
• educate residents and families about advance directives and discharge options
• document interactions in the medical record
• support adjustment to the facility
• coordinate hospice referrals when indicated
• begin and maintain discharge planning, including DME and home care arrangements.
