The Certified Nursing Assistant / Personal Care Aide — PACE role is onsite in Oceanside, California, with a Monday–Friday, 8 a.m.–4:30 p.m. schedule and pay of $22.88–$32.08 per hour, depending on experience. Neighborhood Healthcare PACE supports seniors in maintaining health and quality of life at home and in their community. The role provides person-centered care across the day center and community settings, supporting participants’ safety, independence, daily needs, and engagement. It works with the care team to document observations, assist with personal care and appointments, and support daily PACE operations.

What you'll do

• Record routine tasks and participant care
• document changes and incidents
• escort, lift, and position participants
• reinforce selected teaching initiated by registered nurses
• assist with home visits and activities of daily living
• accompany participants to appointments and services
• provide personal care and supervision
• promote independence and safety
• communicate observations to licensed staff and the interdisciplinary team
• support meals, participant flow, and care coordination.
