What you'll do

• Respond to after-hours calls from staff, participants, and families
• assess symptoms and triage needs
• coordinate with on-call providers and facilities
• provide nursing treatments and education
• support care-plan implementation, proactive calls or visits, and facility transitions
• document care in the electronic medical record
• hand off high-risk participant information to peers and center teams.
