What you'll do

• Advocate for patients and families
• evaluate hospice eligibility and admit as appropriate
• educate patients, families, and healthcare professionals
• coordinate referrals and care transitions
• provide nursing care according to care plans
• observe and report changes
• attend interdisciplinary meetings and outreach events
• assist with compliance reports
• triage calls
• coordinate staff training and orientation
• participate in nursing on-call rotation as needed.
