What you'll do

• Provide primary care to a member panel, including chronic conditions, acute exacerbations, and post-hospital, ER, and SNF follow-up
• lead an interdisciplinary care team
• coach physicians and advanced practice providers
• assess escalation needs
• accept same-day, next-day, and urgent appointments
• monitor trends to reduce ER visits, hospitalizations, and readmissions
• manage transitional care plans
• meet quality metrics
• provide rotating on-call coverage. Approximately 70–80% of time is direct clinical care.
