The MDS Nurse / MDS Coordinator – Skilled Nursing Facility role is onsite in Stoughton, MA, with pay of $45–$50 per hour. The position supports resident care and clinical documentation at The Center at Blue Hills. The coordinator completes accurate, timely MDS assessments, works with the interdisciplinary team on care plans, and helps maintain regulatory compliance and survey readiness. The role ensures resident assessments and records reflect clinical condition, functional status, and care needs.

What you'll do

• Coordinate and complete MDS assessments accurately within regulatory timeframes
• review medical records and clinical documentation for accurate coding and support
• collaborate with nursing, rehabilitation, dietary, social services, activities, and other team members
• participate in care-plan meetings and help develop and update comprehensive plans
• ensure assessments reflect residents’ clinical condition, functional status, and care needs
• monitor schedules and timely submission
• identify and resolve documentation discrepancies with departments
• monitor condition changes and ensure documentation and assessments are completed
• maintain organized MDS records
• support regulatory compliance, survey readiness, and quality-improvement initiatives
• maintain confidentiality and comply with HIPAA and facility policies.

Why this role

Monday–Friday schedule; requires a current Massachusetts RN or LPN license. Previous MDS experience is strongly preferred; RAC-CT or other MDS certification is a plus.
