What you'll do

• Collaborate with the healthcare team on wellness exams and healthy living
• review registries and quality measures
• support primary care workflows and value-based contract goals
• guide patients through care transitions
• introduce eligible patients to TCM, CCM, PCM, and RPM programs
• address social determinants of health needs
• educate patients on chronic disease self-management
• identify changes in condition and report them to providers
• use motivational interviewing and develop patient-centered plans.
