The role promotes consistent workflows and compliance through staff support, chart audits, training, reporting validation, metric analysis, and process improvement across teams and facilities.

What you'll do

• Support CCM and PCM program goals and quality metrics
• oversee daily documentation, accuracy, and productivity standards
• assist coordinators with workflow and charting
• conduct chart audits
• support onboarding and training
• partner with quality assurance on trends and corrective actions
• validate reports and dashboards
• track and present performance metrics
• prepare facility-facing reports and support Director of Nursing outreach
• collaborate with program, QA, compliance, and data analytics teams.
