What you'll do
- Conduct home visits and comprehensive assessments
- administer at-home IVs, change dressings, and clean wounds
- document physician orders and patient status
- initiate and revise care plans with the Clinical Team Manager
- coordinate with the home care team
- complete recertifications, transfers, and discharges as requested
- assess need for other services
- conduct Home Health Aide supervisory visits.
