Respond to patient needs during evenings, nights, weekends, and holidays; serve as the primary liaison between patients, families, physicians, and the hospice team; respond to urgent calls, patient visits, and symptom management needs; evaluate patient and family needs; support and collaborate with an interdisciplinary group; accurately document clinical visits and communications.
