Hospice Registered Nurse Case Manager

The Hospice Registered Nurse Case Manager role is onsite in Ruston, LA; compensation is not specified. The position supports hospice care focused on meeting patients’ and families’ spiritual, social, and clinical needs with dignity and comfort. The nurse coordinates implementation of each patient’s plan of care, assesses and reassesses needs, documents care, and collaborates with families, physicians, and the interdisciplinary team. The role also supervises hospice aides and LPNs and participates in the on-call rotation as needed.

What you'll do

• Coordinate implementation of each patient’s plan of care
• assess and reassess patient and family needs at least every 14 days
• provide nursing services and document goals, interventions, and responses
• collaborate with physicians and the interdisciplinary team
• instruct families in self-care when appropriate
• supervise and evaluate aides and conduct supervisory visits
• maintain clinical notes and personnel documentation
• participate in the on-call rotation as needed.
