The stated salary is $80,000–$89,000 per year, based on experience. Care360 Hospice provides hospice care in patients’ homes and facility settings. The role coordinates individualized hospice care by assessing patients, developing and maintaining care plans with the interdisciplinary group, and communicating with patients, families, physicians, aides, and licensed practical nurses. It helps ensure patient needs are addressed and care changes are coordinated; the schedule is Monday–Friday, 8:00 a.m.–5:00 p.m., with an on-call rotation.

What you'll do

• Conduct initial and ongoing comprehensive patient assessments, including vital signs and medical history
• visit patients at home
• develop and maintain individualized care plans with the interdisciplinary group
• coordinate with patients, families, team members, attending physicians, and the hospice medical director
• perform hospice evaluations and informational visits as needed
• educate patients and families on treatment plans, medication management, and health maintenance
• submit timely, accurate clinical notes
• participate in IDG and case conferences
• perform on-call duties per policy and as needed
• supervise and evaluate hospice aide care at least every 14 days or as needed.
