Clinical Care Coordinator

The Clinical Care Coordinator role is onsite at Amae Health’s Los Angeles (Westwood) clinic; starting compensation is $21.50 per hour. Amae Health provides specialty outpatient care for people with severe mental illness through high-acuity, in-person clinics, with an integrated team of psychiatry, therapy, primary care, dietitians, and community support. The coordinator supports members’ daily experience and clinic operations by facilitating activities, welcoming members, managing communications and records, and sharing observations with the interdisciplinary team. The role helps connect members with community resources and supports coordinated care and engagement.

What you'll do

• Facilitate groups, arts and crafts, and recreational activities
• welcome members
• manage phone and email communications
• assist with scheduling and record updates
• share observations with the interdisciplinary care team
• identify member needs and connect them with community resources
• participate in team meetings, training, and supervision.
