Personal Care Assistant, Cantonese or Mandarin

The Personal Care Assistant role is onsite in San Leandro, CA; compensation is $21.99–$29.20 per hour. The position supports Asian Health Services’ Program of All-Inclusive Care for the Elderly (PACE), which provides coordinated, culturally competent care to seniors in their communities. As a member of the PACE interdisciplinary team, the assistant provides personal care according to participant care plans, supports meal service and transfers, accompanies participants to appointments as assigned, and reports changes in health or psychosocial status. The role also completes required documentation and supports quality improvement activities.

What you'll do

• Provide personal care including grooming, dressing, bathing, toileting, feeding, and ambulation. Assist with meal service and participant transfers
• accompany participants to outside medical appointments as assigned
• monitor and report changes in health or psychosocial status
• attend assigned meetings
• complete PACE EHR documentation
• and participate in quality improvement activities as assigned.
