Registered Nurse/Licensed Practical Nurse (RN/LPN) - Part-Time Evening Shift

Performs professional nursing duties in accordance with facility policies, State Nurse Practice Act, OBRA, and HIPAA. Collaborates with the interdisciplinary team to formulate and update resident care plans. Institutes care protocols for falls prevention, safety, behavior management, skin integrity, etc. Assesses and documents changes in resident status, reporting to physicians.
