Transition Specialist

The Transition Specialist role is hybrid in Saint Louis, MO, with a stated pay range of $19–$22 per hour. The role helps people move from nursing facilities or other institutional settings into safe, accessible community living. Through person-centered planning, transition coordination, resource linkage, documentation, and follow-up, the specialist supports participant choice and long-term stability.

What you'll do

• Provide options counseling, transition support, referrals, advocacy, and independent living skills training
• identify participant goals, support systems, needs, preferences, and barriers
• coordinate with participants, facilities, agencies, families, and support teams
• develop and implement transition plans
• coordinate housing, services, move-in needs, and follow-up
• maintain records and program documentation
• participate in outreach and quality improvement.
