What you'll do

• Interview patients and relatives to obtain social histories relevant to medical care and planning
• help address environmental and social difficulties affecting care
• serve as a liaison among patients, relatives, medical and nursing staff, and outside agencies
• assist with discharge coordination, patient and caregiver assessments, and follow-up
• identify post-discharge risk factors and apply interventions to reduce risk
• connect patients and caregivers with outpatient providers.
