The role supports adult day health care participants through program assistance, supervised personal care, and transportation. It helps maintain safe, organized daily operations and communicates participant needs to the care team.

What you'll do

• Assist with program assignments and participant care under staff direction
• provide supervised personal care
• support activities and meal service
• clean and organize program areas
• document services
• coordinate participant transportation schedules
• transport participants to and from the center
• assist with transfers and ambulation
• maintain vehicle safety, mileage, and maintenance records
• report participant-related information to the team.

Why this role
