What you'll do

• Conduct calls, texts, and email outreach for at least one year after discharge
• track interactions and follow-up tasks in Salesforce or a similar CRM
• support Joint Commission aftercare requirements
• facilitate one weekly alumni group with a licensed therapist
• coordinate and attend one weekly H&I commitment
• review intake forms for accurate contact and emergency information
• build relationships before discharge
• connect alumni with resources
• encourage reviews
• monitor well-being and coordinate re-engagement with admissions and clinical teams.
