Referral Coordinator

What you'll do

• Administer and enter referral orders in an electronic health record system
• track referrals to external institutions and systems
• contact participants and case managers by phone or mail
• prepare referral outcome reports
• coordinate referrals with onsite and offsite providers and programs
• and assist participants with public aid, medical cards, and All Kids insurance as needed.

Why this role
