Inclusion Services Coordinator

What you'll do

• Accompany individuals to community sites for services specified by their individual plans
• help identify community inclusion, self-advocacy, and employment goals
• provide positive role modeling
• maintain timely progress notes and service documentation
• transport individuals to community opportunities
• establish community partnerships and serve as an agency contact
• report unusual incidents and participate in investigations as required
• support regulatory and compliance reviews.

Why this role

Requires two years of experience in the developmental disabilities field, a high school diploma or GED, a valid Ohio driver’s license, reliable transportation, and auto insurance. CPR/first aid, PBS, and medication administration certifications must be obtained and maintained. Schedule may include first and second shifts and some weekends.
