What you'll do

• Provide housing retention services, including client identification, intake, assessment, housing placement, and advocacy
• coordinate housing support, crisis intervention, independent living skills, and health, nutrition, and vocational readiness education
• visit housing sites to check health, safety, and service access
• coordinate with providers and referral sources
• arrange housing requests and site inspections
• participate in case conferences
• track rent contributions and referrals
• connect clients with job readiness resources and eligible benefits
• coordinate with case management staff.
